If you prefer stretched image to black bars - uncheck "Hide desktop background" and "Keep aspect ratio" in settings.

If you want to use different aspect ratio change "Window initial position & size" - 1280x720 = 16:9 aspect ratio.

I like seeing my taskbar but if you want to play in borderless fullscreen select "Desktop" instead of "Desktop work area". If you want it windowed, select "X,Y coordinates".

If you want to run the game from shortcut and not from DxWnd:
Create a shortcut to DxWnd, open properties and add /R:n after dxwnd.exe, where n is index of your game entry in DxWnd. I have only one game there so the index is 1 and my shortcut target ends with dxwnd.exe /R:1. In compatibility tab set it to run as admin and you're done!

If you mean numbers in .dxw file - those are settings for DxWnd. I don't remember which one is responsible for what, I used that program years ago.

It should work without any changes if you have the game on Steam and it's installed to the default path. If you need to make changes - edit them in the GUI of DxWnd.
