This script is a great solution if you're comfortable with Python, and I like that it keeps the original MBOX untouched while extracting attachments. Judging by the comments, it's also been tested successfully on fairly large mailboxes.
One scenario I'd consider separately is when the requirement comes from an IT or legal team rather than a developer. In those cases, maintaining Python environments, dependencies, or custom modifications for different MBOX variants isn't always practical. A dedicated offline extractor can be easier to hand over to non-technical users.
We've used the BitRecover MBOX Attachment Extractor for projects like that. It supports bulk extraction from multiple MBOX files, all major MBOX variants (Thunderbird, Apple Mail, Google Takeout, Opera Mail, etc.), selective extraction by attachment type, date or sender filters, preservation of folder hierarchy, and extraction without modifying the original mailbox.
One feature I particularly like is the ability to extract only specific attachment extensions (for example, PDFs, DOCX, or ZIP files). When you're processing a mailbox with tens of thousands of emails, being able to ignore everything except the document types you actually need can save a significant amount of post-processing compared to extracting every attachment indiscriminately.
